voXel is a DICOM viewer for iPad, written by one person. Mail goes to a real inbox and gets a real reply, usually within a few days.

Before you write, please do not attach medical images. I cannot lawfully receive patient data and I do not want it. Everything below is answerable without it.
voXel is for reference, education and review. It is not for primary diagnostic interpretation, and it must not be the basis of a clinical report or a treatment decision.
The reason is the hardware, not modesty. An iPad panel is not calibrated to DICOM Part 14 GSDF, two iPads will not render the same greyscale identically, and True Tone and Night Shift change it again. Read studies on the system built for it.
Almost always the compression.
voXel decodes uncompressed DICOM and lossless JPEG. It does not decode JPEG 2000, JPEG-LS, baseline or extended JPEG, RLE, deflated, or big endian transfer syntaxes. When it meets one it tells you which, by name, rather than showing you an approximation of your image.
If that is what you are seeing, the fix is at the other end: most PACS can export the same study uncompressed, and that copy will open.
Two other causes worth checking:
Compressed RAR archives are not supported. Stored RAR entries and ZIP archives are. 7z is not.
Some files are simply damaged before voXel ever sees them. A study that was cut short while being copied or burned will be refused with a reason, and no viewer can recover it. Fetching the study again is the only fix.
Those tools are voXel Pro. Viewing, scrolling, window and level, cine playback and the tag inspector are free and stay free.
If you have already bought Pro and it is asking again, open Settings inside the app and use Restore Purchases. That asks Apple, not me, so it works on a new iPad and after reinstalling. If it still does not restore, check you are signed in with the same Apple Account that made the purchase.

Two things in the app will tell us both more than a description can.
Diagnostics, at the bottom of the sidebar, runs the parser's test suite on your device and shows every check pass or fail. The share button on that screen writes a report. If any check fails, that report is the single most useful thing you can send me.
Report a Problem writes a file describing what went wrong, with the app version, your device model and the recent failures. Both are written so that they contain no file paths, folder names or patient details, and you can read the whole file before you decide to send it.
Any of these work:
Copy the folder into the Files app, then import it in voXel with the plus button. Plug in a USB-C drive or card reader and import straight from it. AirDrop a ZIP from a Mac. Or switch on the local transfer server in voXel and open the address it shows in a browser on a computer on the same network, which is useful when there is no cable and no AirDrop.
The transfer server is off unless you turn it on, is protected by a six digit code that changes each time, and works only on your own network. It is not encrypted, so use it at home or in the office rather than on public wifi.
No. It reads files you already have. There is no DICOMweb, no query and retrieve, and no server of any kind.
They stay on your iPad. voXel has no account, no cloud and no analytics, and collects nothing at all. The full explanation is in the privacy policy.
One caution about sharing, because it surprises people: a 3D surface exported as STL or OBJ carries no DICOM tags, so it looks anonymous. It is not. A surface built at a skin threshold from a head scan is a recognisable face. Treat an exported mesh as identifiable unless you cropped it down to something that is not.
Worth asking. Some things are decided and some are not possible, and it saves you the email to know which.
Not possible while voXel is built on an iPad: JPEG 2000 and JPEG-LS, which need C libraries that iPad development does not allow to be compiled.
Not planned: PACS connectivity, cloud sync, and any automatic measurement or detection. voXel contains no algorithm that interprets an image, on purpose.
Open: most other things. Tell me what you were trying to do when you wanted it, which is more useful than the feature name.
